Who are you looking for?
Nurse practitioners, physician assistants and physicians who want more autonomy, more time with patients and a real share of what they build. Experience in primary care, hormones, weight management or functional medicine is a plus, not a requirement.
Which states?
We currently see patients in Virginia, Florida and New York, and we’re expanding into North Carolina, South Carolina, Georgia and Maryland. If you’re licensed in one of those states, we want to talk.
Is this employment or independent?
We’ll match the structure to you. Our goal is to give independent-minded providers the freedom of their own practice with the support of an established one. Specific terms are shared during interviews.
Do I have to bring patients with me?
No. Our marketing, referral programs and the Primary Square marketplace help you build a panel. If you do have patients who want to follow you, we’ll help you welcome them.

Primary Square is a new health app that brings a patient’s information, care team and plan together in one place. Livewell Passport is a founding partner, and our providers are among the first to practice with it.
